The E4 HealthMap:

  • Captures all member-specific psychosocial and health information in an integrated profile.
  • Applies clinical analytic rules to prioritize member risk scoring and care planning.
  • Guides all coaching activity and recording of clinical and financial results for reporting.

The platform applies E4’s evidence-based clinical risk rules to each individual’s data set on a scheduled basis, continuously updating their care needs. A care coach takes this information and coordinates interventions matched to the individual’s unique care needs. The results of the interventions are then tracked and reported as a series of individual and plan sponsor reports based on privacy laws.

Built by industry experts, the E4 HealthMap doesn’t rely on “after-the-fact” claims and demographic data. It goes the extra mile by recording and integrating key leading psychosocial indicators (e.g., risk behaviors/lifestyle, underlying mental health issues, motivation to change) that impact current and future well-being and health of the plan member. Including personal information on self-reported health concerns, motivation to change, health beliefs and undiagnosed psychological disorders adds a significant opportunity for early stage detection and risk management of lost productivity and future preventable demand for medical services. The technology also automates the delivery of preventive care reminders using Centers for Disease Control guidelines as well as coordinates the filling of gaps in care for those with chronic illness.
