Trusted providers who are part of E4Health’s network are strategic and vital partners in our provision of EAP and other behavioral health interventions. Our providers are dedicated to E4′s mission to provide integrated services that not only identify underlying issues, but also provide tools and resources for empowerment and ongoing self-improvement. This allows us to effect individual change and deliver demonstrable results.

Our network providers maintain the following credentials:

Minimum of a Master’s Degree with an active state license in a behavioral health discipline
Three years post–graduate experience working in a counseling environment
Knowledge and access to local resources
Experience in provision of EAP services

  • Copy of your current license(s)
  • Copy of your current liability cover page
  • Copies of any additional certifications
  • Group* only: Roster of Providers with credentials and specialties
