E4’s innovative and highly integrated HealthMap Solution consists of three main programs: EAP, Wellness, and Care Coordination. We developed HealthMap to provide a single platform for population management administration of employee wellbeing programs. E4’s HealthMap Solution leverages behavioral expertise and analytics to engage and empower members to improve their lives and better manage avoidable risk factors.
Most of our clients have over 300 employees; however, we still have several clients with smaller populations, even as few as 100 employees. We will work with you to properly assess your culture and design plans based on your environment and employee needs. E4 targets Fortune 500 companies, those in the public sector as well as retail, pharmacy, higher education, government and Taft-Hartley groups.
Yes, each program within HealthMap can function alone and be bought separately, but the beauty of the solution is the integration. Integrating all data to build a complete member profile allows E4 to assist members in all emotional and physical wellbeing concerns to live a happier, more productive life.
Our HealthMap technology delivers all three systems on one integrated platform. Centralizing data allows E4 Member Advocates (counselors) and E4 Health Coaches to view the member’s complete profile, and assist in all appropriate areas. For example, if a member is working with an E4 Health Coach, and determines wellness issues, such as weight gain, may be a result of anxiety and stress, then the E4 Health Coach can easily transfer the member to an E4 Member Advocate (counselor) to help in the EAP appropriate issues. Our coaches and counselors are trained in motivational interviewing, positive psychology, and mindfulness. Our behavioral approach helps us determine the “why” in the problem, and best assist the member. You can offer members infinite advice and resources, but until core problems are analyzed and addressed, positive, healthy results usually do not emerge. E4’s behavioral approach, focusing on the “why”, helps us to successfully assist members in achieving their emotional and physical wellbeing goals.
Yes, on average our clients receive an estimated ROI from EAP services of at least six dollars saved per dollar spent. We have built a calculator that supports the modeling of ROI using benchmark data or actual data (if available) from the employer. We can run our Impact Calculator on all of our products, separately or together. E4 would be pleased to demonstrate our ROI impact calculator to you for any of our HealthMap Solutions.
E4’s HealthMap Solution repositions program impact by both “responding” and proactively “outreaching” to members’ needs by creating an member profile. This profile, combined with our psychosocial based algorithmic stratifications, enables multiple opportunities for personalized member engagement, ranging from educational emails to clinical outreach phone calls. All of E4’s clinical support interactions are provided by Master’s level counselors, not customer service representatives.
Our utilization report includes key “At a Glance” charts to provide an easy reference and summary of your population’s utilization at both the member and organizational levels along with a summary of key “Presenting Issues.” Additional demographic, impact, and case resolution metrics are detailed herein. All available mediums are included in the report – calls, website triggers, face-to-face sessions, trainings, webinars, management referrals, etc. This information provides data on the challenges your population is facing, clarity regarding E4’s response, and targeted communication opportunities based on real workforce issues and needs.

Built specifically to provide coordinated, comprehensive support in employee assistance, wellness, population health, and other health and productivity associated programs—eliminating duplicated vendor costs. The platform drives the automated integration of multiple data sources, including: psychosocial, health risk, Rx, medical data, lab values, disability and workers comp claims data to support a holistic view of the individual’s wellbeing and health, individualized care needs and outcomes of that care delivery.

E4 HealthMap does not rely on “after-the-fact” claims and demographic data, but goes the extra mile in that it records and integrates the key leading psychosocial indicators (e.g., risk behaviors/lifestyle, underlying mental health issues, motivation to change) that impact current and future wellbeing and health of the plan member. Including personal information on self-reported health concerns, motivation to change, health beliefs and undiagnosed psychological disorders adds a significant opportunity for early stage detection and risk management of lost productivity and future preventable demand for medical services. The technology also automates the delivery of preventive care reminders using Centers for Disease Control guidelines as well as coordinates the filling of gaps in care for those with chronic illness.  .
The platform applies E4’s evidence-based clinical risk rules to each individual’s data set on a scheduled basis to continuously update their needs for care. A care coach takes this information and coordinates interventions matched to the individual’s unique needs for care. The results of the interventions are then tracked and reported as a series of individual and plan sponsor reports based on privacy laws. Examples of rules include: identification of risk for mental health issues, unhealthy behaviors, and gaps in care. Rules are embedded to match licensed provider to identified risk by specialty and care plans linked to risk.
E4 HealthMap supports the collection of confidential survey data whether from the short form LifeReach survey used in connection with our employee assistance programs (LifeScope) or the more comprehensive emPower survey used in conjunction with our wellness (LifeChoice) or chronic care management (LifeLync) programs. These surveys are completed by eligible members that provide data on psychosocial and lifestyle related health risk. When eligible members of a plan complete emPower, the data files are exported into E4 HealthMap. This critical information is then integrated with other data (e.g., medical and prescription drug claims, biometric data, lab values, disability and workers compensation claims) and is translated into a comprehensive member-centric record useful for risk scoring. Coaching interventions can also be triggered solely from survey data even without the integration of claims data.
